Assessment & Pathways Officer (Part time)

Hours: 28 per week, days negotiable

Full time salary £26,007, pro rata for hours worked

Location: Uxbridge

We are looking for an experienced and motivated individual to lead on the development and delivery of our young carers assessment and pathways function. This is a key role within the organisation, responsible for ensuring that young carers receive high-quality assessments and are supported through clear, effective pathways tailored to their needs. You will work closely with colleagues across our service and with external partners to ensure a coordinated approach to support.

‍ ‍Main duties include:

  • Coordinating the development, implementation and review of assessment tools and processes

  • Undertaking and overseeing initial and review assessments of young carers

  • Ensuring accurate and compliant recording of information on our CRM system

  • Developing individual support pathways based on assessed need

  • Working collaboratively with internal teams to ensure appropriate support across services

  • Supporting multi-agency working and contributing to case discussions and conferences

  • Monitoring outcomes and contributing to reports for funders

  • Reviewing and improving service delivery through continuous feedback and evaluation

  • As part of the assessment, outreach may be required

About you

You will:

  • Have experience of working with children, young people or families in a health, social care or community setting

  • Have experience of assessment, support planning or case management

  • Be confident working both independently and as part of a team

  • Have strong organisational and communication skills

  • Be committed to safeguarding, equality and person-centred practice

  • Be flexible and able to respond to the changing needs of the service
